• To work Friday, Saturday and Sunday and Bank Holidays.
• Friday - based on-site.
• Sat and Sun - mainly remote working.
• Candidate should have experience of Hospital Discharge in line with Discharge to Assess.
• Knowledge and skills in line with Care Act responsibilities.
